Home to Home helps families with eldercare issues that arise in connection with the senior's living environment and care. By developing eldercare plans, a "roadmap" is created to enable families to prepare for changes as the senior's health evolves.

Home to Home offers several categories of services: In-home care and care plans, Housing advice and support, Transitional services, and a post-move service called Peace of Mind. In addition, Home to Home helps families develop eldercare plans to map out their options relating to eldercare and living environment needs. These plans allow families to be prepared and know where to turn when unexpected changes occur.

In-Home care and care plans

Home to Home can help seniors continue to live at home safely and comfortably with the right support. By partnering with a leading home support business, Home to Home can develop an in-home care plan which provides the custom assistance that the senior needs, and deliver that service with the best quality management possible.

Seniors Housing Advice and Support

Home to Home eliminates the time-consuming research, confusion and stress involved in selecting a seniors residence. With our personalized services, an assessment is conducted with respect to various elements of the senior's lifestyle. Details of the recommended homes are reviewed, shortlisted, and appointments are made to tour the suitable residences. Home to Home provides the support and expertise in the selection process to ensure that the senior is making an informed choice.

In addition, if the senior is needing to sell his or her current home, Home to Home can refer the senior to one of a select group of real estate agents who can assist in the sale of the current home.

Transitional Services

After the new residence is selected, the work has only just begun. There is a considerable amount of planning and organization required to complete the move. Our transitional services involve assistance with downsizing and disposal of items, finding movers, packing and unpacking, re-routing services, supervision on the day of the move, and sending out notifications to friends and family.

Each move is unique, and an individualized plan and timeline will be developed with each client to address each element of the move.

Peace of Mind

Peace of Mind is a post-move ongoing monitoring and advocacy service, aimed at benefitting family members of the senior. Perhaps not all the family members live close by, or perhaps they would simply prefer not have to deal with all of the administrative issues that arise from time to time.

Just because the senior has completed the move does not mean the family members stop worrying about them. With Peace of Mind, regular visits are made to the senior, and reports provided to the family members through a secure web based program. It allows all the family members to not only converse with Home to Home, but also with each other. Through this program, a history of communications and files relating to the senior can be kept over time.

Peace of Mind provides unlimited local support in answering questions that family members may have on an ongoing basis. By having all of the updates, information, Q&A and tasks relating to the senior stored all in one program, family members can stay up to date with what's going on and be sure that they are all operating under the same information.

As issues arise regarding the services and care that the senior is receiving in his or her new home, Home to Home can act as an advocate for the family in facilitating solutions. We want you to consider us as an extension of your family.
